I'm an agent and I've received a survey invitation, what do I do?

The survey invitation is only for the person named in the email or text message you received. If you are that person’s agent, or other form of representative, the platform has been provided with your email or phone number as their contact point. Please forward the invitation directly to them. 

To keep their survey answers confidential they will be asked to use their own email address when saving their responses. This address will be kept confidential and used only by the platform to provided them with future access to the information they have provided and to contact them for the limited purposes described. 

Every question in the survey is optional.
